Community Aged Care Program

The Community Aged Care Program Provides services and support to frail elderly people who are assessed as needing regular help, but would much prefer to remain in their own home.

How to apply for a package
First contact your local Aged Care Assessment Team. A member of the team will visit you at home to find out what your needs are.
They will determine whether you are eligible for a care package or if there are other services that may be more appropriate for you. Within funding limits the type of service is tailored to the individual client to best suit their needs to help them remain in their own home. These may include:
* Personal hygiene and dressing
* Getting in and out of bed
* Preparing and eating meals
* Transport to shops, bank, doctor or social gatherings
* Rehabilitation
* Home safety and security
* Household tasks, e.g. cleaning, laundry, gardening & shopping
