Top level Community Health Services Aged Care Assessment Team


The Aged Care Assessment Team (ACAT) can provide assessment, information, advice and assistance to older people who are finding it difficult to manage at home or who are thinking about moving to supervised accommodation.

AGED CARE ASSESSMENT TEAM

The Team accepts self referrals or referrals from family, carers or health professionals. However, it is important that the person concerned be aware of the referral and agree to have contact with the Team.

The assessment can occur in the person’s home so that the person’s needs can be determined. Recommendations and information about the following options will be given:

  • Low care (previously Hostel)
  • High care (previously nursing homes)
  • Community aged care packages
  • Independent living units
  • Remaining at home with support services
