To make a referral phone 8552 0600. Depending of the type of request it may take up to seven days for a health worker to contact you.

All information is treated confidentially.
Referrals are taken from individuals (self referrals), relatives, health service providers (i.e. G.P’s and Hospitals) schools and other community organisations. A doctor’s referral is usually not necessary.
Referrals from G.P’s or other health services require a completed Southern Fleurieu Health Service Request form faxed to Southern Fleurieu Health Service on 8552 0616. Please contact the Health Service if you require these forms.
