This is a government funded program set up to provide services to clients receiving other assistance from Southern Fleurieu Health Service and the Community Nurses.
|
The aim of the program is to support people to stay in their own homes for as long as possible prior to a supported care accommodation. The CSS Co-ordinator assesses individuals in their own homes then plans the most appropriate service with the most suitable contractor for the job.
Services CSS can provide are:

Personal Care (showering, dressing, shaving)

Respite sitting (to give Carers a break)

Support /check visits (for people living alone, needing daily supervision)

Transport and assistance to attend medical appointments

Meal Provision (Where Meals-on-Wheels are unable to meet individual’s needs)

Home Help (to young chronically ill and disabled people)
