The Community Nursing Service provides comprehensive nursing care to people of all ages in the home and in the community.

Nursing care is delivered by a team of highly skilled and experienced nurses.

Who can make a referral?

Referrals are accepted from:

* The client

* The client’s family

* Discharging hospitals

* General Practitioners.

Referral process:

Simply phone 8552 0649 Mon-Fri 0800-1600

All referrals are taken over the phone. Please have all details handy including Medicare and pension numbers.

Services provided:

ASSESSMENT AND CARE PLANNING

After a referral has been made to our service the client will be contacted by phone and a suitable time will made to visit. On the first visit a full nursing assessment will be undertaken and an appropriate care plan formulated in consultation with the client.

WOUND MANAGEMENT:

A comprehensive assessment of the type of wound is made and the client is educated on ways to prevent reoccurrence.

Types of wounds include:

* Acute and chronic leg ulcers

An assessment is made to determine if the ulcer is venous, arterial or mixed. One way of doing this is by using a Doppler. Results will be forwarded to your GP. If the ulcer is venous in nature you will be required to wear compression stockings. The first stocking is provided by CNS and future stockings can be purchased from your nurse.

* Skin grafts

all types including split skin grafts and rotation flaps.

* Skin tears

* Pressure ulcers

* Wound drains

* Complex wounds sinuses, fistulas, dehiscence etc.

* Burns

STOMAL THERAPY

Short term support, advice and education can be given to clients following discharge from hospital. This builds on the information already received in the hospital and allows clients to care for their stoma independently.

Includes:

* Colostomy

* Ileostomy

* Urostomy

* Fistulas and sinuses.

MEDICATION MANAGEMENT:

The community nurse is able to assist with many different types of medication administration. Clients are educated so that they can undertake self care.

Types of medication administration include:

* Dosettes/Webster packs:

Clients may be referred on to their pharmacist for a Webster pack or a dosette box may be more appropriate.

Other types of medication administration:

* Injections

* Epidurals

* Portacaths

* Baxter infusors

* Computer Assisted Delivery Devices (CADD),

* Patient Activated Reservoirs (PAR)

* Peripherally Inserted Central Catheter (PICC) management

* Topical medication

CONTINENCE ADVICE

Marg Nitschke, Continence Nurse Advisor (CNA) is based at Mt Barker but visits Southern Fleurieu on a fortnightly basis.

Marg undertakes a full continence assessment (bowel and bladder) and physical exam as well as providing education on all aspects of continence including:

* Pelvic floor exercises

* Accessing continence schemes

* Catheter care and maintenance

* Ultra sound assessment of residual urine

* Liaison with urologists

* Management of suprapubic catheters

* Sourcing stock supplies including for DVA clients

* Education on self catheterisation

NURSING CLINICS

If you prefer to be given an appointment time (with no waiting times), you may like to visit one of the two Nursing Clinics. Please discuss this with your nurse. The Community Nursing Service operate two nursing clinics:

Goolwa: Open Door Centre, 1 Collingwood Street, Goolwa

Victor Harbor: Southern Fleurieu Health Service, Harbour View Terrace, Victor Harbor.

OXYGEN REVIEWS

CNS are currently undertaking in home assessment of equipment for clients on oxygen therapy on behalf of South Coast District Hospital.

Referral for home oxygen must be via a Respiratory Physician.

Home assessment includes:

* Oxygen saturation levels

* Liaison with your Respiratory Specialist

* Safety check

* Information on oxygen concentrators and bottled oxygen

* Ongoing education and support

CONTRACTED SERVICES

The Community Nursing Service provides the following contracted services:

Department of Veteran’s Affairs: provide full range of nursing services to entitled Veteran’s.

Metro Home Link packages: packages of care provided to facilitate early discharge from Flinders Medical Centre, and Noarlunga Hospitals.
